Family Health Centers of San Diego is looking for a Physical Therapy License Applicant to work under the direct and immediate supervision of a Physical Therapist. The Physical Therapist License Applicant provides evaluation and treatment for pediatric or adult patients with a variety of mobility, motor control and musculoskeletal disorders. This position will also focus on prevention by developing and providing early intervention programs/services.
Responsibilities:
- Under the direct and immediate supervision of a Physical Therapist, Communicates regularly with Referral, Case Management, Care Coordination, and Scheduling staff. Makes recommendations and follows through with referrals to outside agencies when appropriate. Assist with case management and referrals to external agencies, occupational therapy, and speech language pathology.
- Communicates with patients and/or parents/caregivers regarding evaluation results, treatment plan and goals. Provides home program activities to patients and/or parents/caregivers to ensure carryover to the home environment. Engages patients and/or parents/caregivers in treatment sessions.
- Under the direct and immediate supervision of a Physical Therapist, completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, documentation complies with patients' insurance requirements.
- Under the direct and immediate supervision of a Physical Therapist, Consults with Physicians, allied health professionals, and other team members. Participates in case conferencing meeting and in program planning meetings. Facilitates training and presentations for staff, patients, and parents/caregivers when applicable.
- Under the direct and immediate supervision of a Physical Therapist, evaluates patients referred for physical therapy, physical therapy consultation, and/or early intervention services (for pediatric assignments). Performs evaluation according to each patient's needs, physician recommendations, and specific program guidelines/protocols.
- Monitors and manages own schedule. Communicates with scheduling staff to keep schedule full.
- Under the direct and immediate supervision of a Physical Therapist, provides treatment at different clinic sites, off-sites, or patient homes based on caseload and department needs. Plans effectively for efficient use of time and resources when providing home visits.
- Under the direct and immediate supervision of a Physical Therapist, provides treatment to assigned patients as appropriate in accordance with established standards of care and practice. Designs an appropriate and effective treatment plan and work within scope of practice at all times. Monitors progress of treatment goals through periodic observations and assessments. Conducts re-evaluations when indicated.
- Performs other duties as assigned.
Requirements:
- Graduation from an approved physical therapist education program required.
- Letter of authorization and completion of application for licensure with Physical Therapy Board of California required.
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
- Ability and means to travel as needed in a timely manner within San Diego County. DRIVER REQUIREMENTS: Requires an active Class C California driver's license, proof of liability insurance at $100,000, on vehicle used. No more than 2 points in past 12 months; No suspensions in last 2 years for moving violations; No DUI, reckless or felony Driving within 5 years. No license revocation in 7 years.
- Ability to perform patient evaluations, synthesize patient information and determine whether the problems to be addressed are within the scope of practice for a PT.
- Ability to provide appropriate interventions based on clinical judgment about diagnoses and prognoses, and based on patient/client goals.
- Ability to use professional literature, evidence-based research, and continuing education content to make practice decisions.
- Basic computer literacy (i.e., use system to manage and schedule appointments, access electronic medical record information).
- Bilingual English-Spanish is preferred.
- Excellent interpersonal and customer service skills.
- Intermediate written and verbal communication skills.
- Knowledge of appropriate treatment strategies for patients in outpatient rehabilitation settings.
- Knowledge of Physical Therapy principles, practices, evaluation, and treatment techniques, including knowledge of early intervention services (for pediatric assignments).

34.32 - 48.98If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
